Nursery Application Form Parent Agreement Form 

Filling out the Parent Agreement form, please fill out:

• Section 1
• Section 2
• Section 3 - Part A (Only fill Part B if your child attends another Early Years Setting as well as Woodlands Park)
• Funding Period - Autumn, Spring and Summer Terms
• Section 4 

Please ensure that when submitting your child's nursery application form, you provide two proofs of address, the child's birth certificate, and the parent agreement form. Admission forms must be submitted to the Office or sent via email to admin@woodlandspark-nur.haringey.sch.uk. 

You will be contacted to confirm availability of the place requested and a start date. All families receive a home visit prior to starting at Woodlands Park Nursery this is an essential part of the settling in process and the beginning of your relationship with the key person or teacher.

For 3 and 4 year old children eligible for 30 hours free early education the office will need the HMRC code and we are happy to give you advice or help in applying.
